Do you know a member of our community that has made a huge change to live a moreheart-healthy lifestyle?Have they started eating healthier, getting more active, or encouraging their friends and family to get heart healthy with them? 2016 Lifestyle Change Award Winner


The American Heart Association is taking nominations for the Lifestyle Change Award! This is not just about getting in good physical shape; it is about improved self-esteem, recognizing positive changes and establishing healthy habits for life. Find the application form here!


We want to recognize individuals who have made positive changes to improve their quality of life and health, and influenced those around them to do the same. No change is too small; every accomplishment is significant and should be celebrated!

Nominations can be submitted by friends, co-workers, relatives, or individually. One winner will be chosen by a selection committee and announced at the San Luis Obispo Heart & Stroke Walk on Saturday, September 16th, 2017.
