The Coumadin Clan includes a group of clinical ambulatory care pharmacists, medical assistants, and patient service representatives who serve the Rhode Island community members who are on blood-thinning medications. This includes the cardiology, primary care, and pulmonary practice settings. Our role includes patient care for those on warfarin/DOACs for various disease states, including those who have experienced stroke or other heart health related concerns. My colleagues and I have started a team for the upcoming Heart Walk and appreciate any support to this cause! We are very aware of the impact heart disease has on families and loved ones and strive to assist with compassion and due diligence.
